Health care task force update.
>> Great. How are you? Okay. Uh, we're going to give you an update today. It's Neil and I have been working um along with a lot of other groups to try to put together the rural healthcare model. But I want to address a couple things first.
I want to address wear. Um this has not been a working relationship. So I will start out with that. Um until we sent a letter over to the wear board. We hadn't heard from them. I reached out to you to try to get on some of these meetings. I got completely ignored.
Not even a you know I sent one note back and then I said, "Well, when can I come?" And then you just posted me. So you know, if we're going to work together, we're going to work together.
But I will tell you that when you when we get things like this that says a proposal to bring convenient care to wear and has nothing to do with Harwick or Gilbertville or anything else. You can just see it has nothing to do about us. What this is is a Gilbert trust. We as a town
Mary Lane was in the control of where sold Mary Lane. Mary Lane and the whole thing with the way we went. We didn't have any control over that. Okay. The way the zoning that's not our call, none of us could vote on this. We could tell our friends what to do or you know
Suggest things. We had no control over how that got zoned. But the Gilbert trust is ours.
Ours. And we have been completely blocked from all of those discussions.
And that is not right because this money needs to help this community as well as your community. And you guys coming up with some great ideas of what may help you is not what may help us. We have a very different community. We are a true healthcare desert by the way. We are 35
To 45 minutes out. You are not. You have wings. You have doctor's offices. You have all kinds of things. We don't have an ambulance. We don't have an EMS. We don't have any of those things. So our needs are different and this trust needs to support our community too and being
Blocked from these discussions with ACE with Bank of America on money that we share from our own Gilbert. You told us and the Gilbert building you know Mr.
Gilbert left this to us and you us.
So, I would I'm not being mean or mad, but I am mad because now, you know, things are happening and we still have been left out. And had they not sent that letter, thank you very much, you would not have shown up. And you know what? That's not the way it should have been. Once you got to the trust, the
Trust is for both of us. It's a $7 million trust plus, by the way. Kicks off a certain amount per year. If you talk to financial people, it's actually about $500,000 a year, not $300,000 a year. So, don't believe what they're telling you. Um, you know, all you got to do is general CPA math on that one.
And and what are they going to use that for? and how can we slip this to make sure that we can set up things that we've been working with with our committee which is can we have a miniature um urgent care that has a triage that allows us to cut down our ambulance calls when people fall down
And actually leverage our north field because we don't use wear ambulance we don't have a fire department we have to leverage our resources really smart here with a really good rural healthcare model and we need some of this seed funding to be able to make that have it.
So you taking it away and going to some pediatic care that's not a urgent care that's you know just a doctor's office does not meet our needs and you wouldn't know that because you haven't talked to us. So, I'm just asking three things from you and whoever's you have anybody

Think we need to be um at the seat at that table because we have as much as states as you do. Second, I think we also have to be in the meetings with Bay State cuz by the way, I'm a healthcare executive. You got a doctor here. We got doctors here. We actually built rural healthcare models all over the country.
We know how to work with Bay State and put together models that make sense, not have them dup you with this type of model. This is not going to work. By the way, the year on that was ridiculous.

Building a model that works for both of us that we can leverage things that we can build something powerful here. We don't have to take what they say serves us. It's not the way it has to be. It's just you got to have a model and know what you need. You you don't they can't dictate to you. You can dictate to them.
This is your trust. It's our trust. this is our money, not their money. So that's all I'm going to say on that part. Now we go to the happy part.
The happy part is uh you know we are a healthcare desert out there. You guys know that and I don't know if all you know but we have been rumaging around in all these buildings out here trying to find that 2500 square ft of perfect land. We might be coming your way guys.

So we are looking for that 2500 ft of perfect land. Um, because we do want to look at something like a public partner, private private public partnership, um, to be able to get that nonprofit in there and to be able to do some type of triage things with the emergency care. Uh, be able to when
People fall, we actually have a crew that can go out and do that stuff versus always throwing these ambulances out there. Um we're uploading pharmacists once a week. they can go out and do those kind of uh um reviews with patients um and actually mail their prescriptions to them and then do the
Reviews with them through telemy tele I mean we talk sort of thing have a base where you can build rural eye care on a routine basis um and um mamography and other needed things that need a train
>> right that need a base there needs to be some >> some infrastructure structure and a base and and we can't we don't need to build that tomorrow with a finger snap, but we need to say this is our our vision. This is our goal. This is how what we want to work to and create some steps so that
Maybe we take some of the easy steps that build up more financial support and move forward to build out the rest of what the town needs.
>> We have a little bit of advantage over that's because we're in Worcester County, right? So, um, we while we're going to be asking Bay to perhaps drive, we're not going to ask for help in the infrastructure. We may ask to leverage some of their tele medicine programs or their pharmacy programs. That will cost
Them a lot of money and make a lot more money in the trust. We can leverage some of their programs and then turn around to UMass and say, "Hey, help us build an urgent care center and do a public private partnership and grant." Um, so we can actually leverage both sides.
Where doesn't have that opportunity, then again, you have a hospital, you have funds, you have all that stuff there. So, there's a lot of stuff we need that we don't have. Um, but we can actually leverage both sides and and that's certainly something we're looking at. What Barry does today, I don't know
If people are familiar with that, they actually leverage residents from UMass to stop that. And that is obviously an opportunity for us as well. Now, they do a lot of mental health um, uh, substance abuse type stuff. So, that's not really a viable alternative for us on that
Front except for that part comes the business model or the health model. Um, but we have actually laid it all out. I was going to give you posters and everything, but I only had 10 minutes.
But I just wanted to make sure that you guys knew we have this thing moving along. We've talked to all parts of the and we were coming for you guys next and Northfield. So, um, to do different pieces of it where the community can start to help out in some of these things, get more people trained as
Paramedics, do some of this training and position people around town that can help support our community because we're not going to become less rural anytime soon. Rural rural, we don't we already lost everything. So, we've got to start building this back up again. Any questions? Yes. Um, the the trust. Yes.

For the alternatives and where we were going to go. For example, there were some stuff that was budgeted placed some soft costs on Lockard, our bond council that we might be able to get reimbursed for. So, the accountant was able to come up with um some invoices from a couple years ago um sent
To uh USD to get their approval. So, this number on the first page um could change the contingency money of all three projects.
So what we are looking at for contingency funds is on the second page and it is a modular office foundation estimate at 75,000 and a surface area FFD
And then the alternate number three which is the sludge basement upgrades.
It's just Gilbert Millville updating the basement with some new piping and >> some new pumps.

Well, to answer part of your question on on the rip wrap that they're putting in around that pump station is cuz they didn't calculate or realize how much rainwater runoff comes off of that slope, right? It takes that loose loom that they put down and just washes away.
I get that. I just designed it as well.
But the bottom line is it's the engineer who designed it. It's the engineer was supposed to be the one overse.

Parts.
Rick Degan who records our meetings has been was instrumental in helping me as I am not an IT expert. Um so he was fantastic to work with during that process.
Working with the town accountant and treasurer on town key cards. We held our prior weekly financial management planning meeting. Met with the treasur and select and tinker on ongoing financial issues. had a Zoom meeting with the town accountant, treasure collector, sewer superintendent, USDA,
And DCP on the finances for the sewer project.
I'm still going over performance valuations with department heads. I think I just have a library left, which is fantastic.
We are advertising for an assistant treasure collector, a wastewater treatment plant operator, a heavy equipment operator, a town accountant, a police officer, and a van driver. Um, I references today for a wastewater treatment plant operator. So tomorrow I will be off offering the position
Pending the select approval. Um had a department head meeting where we discussed the new AP schedule. Insurance claims time off upcoming training. We are switching our AP to do every other week. So payroll and payables are not due in the same week. We're not draining our accounts weekly. It's not extra
Pressure on the treasur and the accountant. And that way it also it's just I've never seen it done both in the same week. Um, so this is just more of a standard operating procedure.
Met with the new police chief and police lieutenant on roles and responsibilities. Uh, I attended an event at the Barry Senior Center. I attended a capital planning committee and a finance committee meeting by phone. Had a call with the supervisor of records through the secretary of the
Commonwealth and our various complaints.
I have a couple people I want to recognize. I already recognize Mr.
Degan, who was really helpful getting the camera situation rectified, but I also want to recognize Chief Kevin Landee on a smooth transition for the process. I always knew he was great to work with, but just seeing him already jump into action. He already had a department meeting with the officers.
He's been, you know, he brought the new lieutenant to the department head meeting and he's just been, it's been really great to see him thrive and just see him um just the transition has been so smooth with the chief retiring and him taking over. So, I just deserve to be recognized for that.
A few important dates. Our public records law training at Eagle Hill School is August 18th from 4 to 6 for all staff and elected officials. I'll be sending out more reminder emails about RSVPing because we have to I just want to make sure we have enough material and national night out at Quad Regional

Um so we're going to as as [00:02:11] an informational session we we have this [00:02:14] panel of three experts hydraologist an [00:02:17] epidemiologist um public health um [00:02:20] expert from Boston College and um the [00:02:24] ex-president of the Massachusetts [00:02:26] Association of Health Boards to help [00:02:28] advise us as to how to move forward. It's [00:11:34] uh Neil and I have been working um along [00:11:37] with a lot of other groups to try to put [00:11:38] together the rural healthcare model. But [00:11:40] I want to address a couple things first. [00:11:42] Um uh I want to address wear. Um this [00:11:45] has not been a working relationship. So [00:11:48] I will start out with that. Um until we [00:11:50] sent a letter over to the wear board. We [00:11:52] hadn't heard from them. I reached out to [00:11:54] you to try to get on some of these [00:11:55] meetings. I got completely ignored. Um [00:11:59] not even a you know I sent one note back [00:12:01] and then I said, "Well, when can I [00:12:02] come?" And then you just posted me. So [00:12:05] you know, if we're going to work [00:12:06] together, we're going to work together. [00:12:08] But I will tell you that when you when [00:12:10] we get things like this that says a [00:12:12] proposal to bring convenient care to [00:12:14] wear and has nothing to do with Harwick [00:12:16] or Gilbertville or anything else. You [00:12:19] can just see it has nothing to do about [00:12:21] us. What this is is a Gilbert trust. We [00:12:25] as a town [00:12:27] Mary Lane was in the control of where [00:12:31] sold Mary Lane. Mary Lane and the whole [00:12:34] thing with the way we went. We didn't [00:12:37] have any control over that. Okay. The [00:12:40] way the zoning that's not our call, none [00:12:43] of us could vote on this. And we have been completely [00:12:59] blocked from all of those discussions. We are a true [00:13:18] healthcare desert by the way. We are 35 [00:13:20] to 45 minutes out. You are not. You have [00:13:23] wings. You have doctor's offices. You [00:13:26] have all kinds of things. We don't have [00:13:27] an ambulance. We don't have an EMS. We [00:13:29] don't have any of those things. So our [00:13:31] needs are different and this trust needs [00:13:34] to support our community too and being [00:13:38] blocked from these discussions with ACE [00:13:41] with Bank of America on money that we [00:13:44] share from our own Gilbert. You told us [00:13:47] and the Gilbert building you know Mr. [00:13:49] Gilbert left this to us and you us. [00:13:54] So, I would I'm not being mean or mad, [00:13:57] but I am mad because now, you know, [00:13:59] things are happening and we still have [00:14:01] been left out. And had they not sent [00:14:03] that letter, thank you very much, you [00:14:05] would not have shown up. And you know [00:14:07] what? That's not the way it should have [00:14:09] been. Once you got to the trust, the [00:14:12] trust is for both of us. It's a $7 [00:14:14] million trust plus, by the way. Kicks [00:14:17] off a certain amount per year. If you [00:14:19] talk to financial people, it's actually [00:14:21] about $500,000 a year, not $300,000 a [00:14:24] year. So, don't believe what they're [00:14:25] telling you. Um, you know, all you got [00:14:28] to do is general CPA math on that one. [00:14:30] Um, and and what are they going to use [00:14:32] that for? and how can we slip this to [00:14:34] make sure that we can set up things that [00:14:36] we've been working with with our [00:14:37] committee which is can we have a [00:14:39] miniature um urgent care that has a [00:14:42] triage that allows us to cut down our [00:14:44] ambulance calls when people fall down [00:14:46] and actually leverage our north field [00:14:49] because we don't use wear ambulance we [00:14:51] don't have a fire department we have to [00:14:54] leverage our resources really smart here [00:14:56] with a really good rural healthcare [00:14:58] model and we need some of this seed [00:15:01] funding to be able to make that have it. [00:15:03] So you taking it away and going to some [00:15:06] pediatic care that's not a urgent care [00:15:08] that's you know just a doctor's office [00:15:10] does not meet our needs and you wouldn't [00:15:12] know that because you haven't talked to [00:15:13] us. Um we're uploading um uh [00:17:48] pharmacists once a week. they can go out [00:17:50] and do those kind of uh um reviews with [00:17:53] patients um and actually mail their [00:17:56] prescriptions to them and then do the [00:17:57] reviews with them through telemy tele [00:18:01] I mean we talk sort of thing have a base [00:18:04] where you can build rural eye care on a [00:18:07] routine basis um and um mamography and [00:18:12] other needed things that need a train [00:18:15] >> right that need a base there needs to be [00:18:17] some [00:18:18] >> some infrastructure structure and a base [00:18:20] and and we can't we don't need to build [00:18:23] that tomorrow with a finger snap, but we [00:18:25] need to say this is our our vision. This is how what we want to [00:18:30] work to and create some steps so that [00:18:33] maybe we take some of the easy steps [00:18:35] that build up more financial support and [00:18:38] move forward to build out the rest of [00:18:40] what the town needs. That will cost [00:18:59] them a lot of money and make a lot more [00:19:01] money in the trust. We can leverage some [00:19:02] of their programs and then turn around [00:19:04] to UMass and say, "Hey, help us build an [00:19:06] urgent care center and do a public [00:19:08] private partnership and grant." Um, so [00:19:10] we can actually leverage both sides. [00:19:12] where doesn't have that opportunity, [00:19:13] then again, you have a hospital, you [00:19:15] have funds, you have all that stuff [00:19:16] there. So, there's a lot of stuff we [00:19:18] need that we don't have. Um, but we can [00:19:21] actually leverage both sides and and [00:19:23] that's certainly something we're looking [00:19:25] at. What Barry does today, I don't know [00:19:26] if people are familiar with that, they [00:19:29] actually leverage residents from UMass [00:19:31] to stop that. And that is obviously an [00:19:34] opportunity for us as well. Now, they do [00:19:36] a lot of mental health um, uh, substance [00:19:40] abuse type stuff. So, that's not really [00:19:42] a viable alternative for us on that [00:19:44] front except for that part comes the [00:19:46] business model or the health model. Um, [00:19:49] but we have actually laid it all out. I [00:19:50] was going to give you posters and [00:19:52] everything, but I only had 10 minutes. Um [00:46:43] Rick Degan who records our meetings has [00:46:47] been was instrumental in helping me as I [00:46:49] am not an IT expert. Um so he was [00:46:52] fantastic to work with during that [00:46:53] process. [00:46:55] Working with the town accountant and [00:46:56] treasurer on town key cards. We held our [00:46:59] prior weekly financial management [00:47:00] planning meeting. Met with the treasur [00:47:03] and select and tinker on ongoing [00:47:04] financial issues. had a Zoom meeting [00:47:07] with the town accountant, treasure [00:47:08] collector, sewer superintendent, USDA, [00:47:09] and DCP on the finances for the sewer [00:47:12] project. [00:47:14] I'm still going over performance [00:47:15] valuations with department heads. I [00:47:17] think I just have a library left, which [00:47:19] is fantastic. [00:47:20] We are advertising for an assistant [00:47:22] treasure collector, a wastewater [00:47:23] treatment plant operator, a heavy [00:47:25] equipment operator, a town accountant, a [00:47:27] police officer, and a van driver. Um, I [00:47:31] references today for a wastewater [00:47:32] treatment plant operator. So tomorrow I [00:47:35] will be off offering the position [00:47:37] pending the select approval. Um had a [00:47:41] department head meeting where we [00:47:42] discussed the new AP schedule. Insurance [00:47:44] claims time off upcoming training. We [00:47:47] are switching our AP to do every other [00:47:50] week. So payroll and payables are not [00:47:52] due in the same week. We're not draining [00:47:54] our accounts weekly. It's not extra [00:47:55] pressure on the treasur and the [00:47:57] accountant. And that way it also it's [00:47:59] just I've never seen it done both in the [00:48:02] same week. Um, so this is just more of a [00:48:05] standard operating procedure. [00:48:09] Um, met with the new police chief and [00:48:12] police lieutenant on roles and [00:48:14] responsibilities. Uh, I attended an [00:48:16] event at the Barry Senior Center. I [00:48:18] attended a capital planning committee [00:48:20] and a finance committee meeting by [00:48:22] phone. Had a call with the supervisor of [00:48:25] records through the secretary of the [00:48:27] commonwealth and our various complaints. [00:48:29] I have a couple people I want to [00:48:31] recognize.
